Assistant Director of Nursing (ADON) – Home Health Care


Key Responsibilities:


Nursing Staff Management

  • Supervise and support nursing staff, including RNs, LPNs, and caregivers.
  • Provide leadership, coaching, and performance oversight to ensure quality patient care.
  • Assist with hiring, onboarding, evaluations, corrective actions, and retention of nursing staff.
  • Serve as a clinical resource and mentor to team members.

Scheduling & Staffing

  • Manage and coordinate nursing schedules to ensure appropriate staffing coverage for all patient needs.
  • Address callouts, schedule adjustments, and staffing emergencies.
  • Collaborate with staff to maintain efficient scheduling and continuity of care.

Care Plan Oversight

  • Review, update, and monitor individualized patient care plans.
  • Ensure care plans are implemented accurately and meet physician orders, patient needs, and agency standards.
  • Monitor patient outcomes and communicate changes in condition to appropriate parties.
  • Support coordination of care among patients, families, physicians, and interdisciplinary teams.

On-Call Responsibilities

  • Serve as the primary on-call contact for all clinical and operational matters two (2) days a week.
  • Respond promptly to urgent staffing, patient care, and employee concerns after hours and on weekends as needed.
  • Coordinate emergency staffing coverage and provide guidance for clinical situations.

Compliance & Quality Assurance

  • Ensure compliance with state, federal, and accreditation regulations related to home health care services.
  • Assist with audits, incident reporting, quality improvement initiatives, and policy implementation.
  • Monitor documentation for accuracy, timeliness, and compliance standards.

Qualifications

  • Current Registered Nurse (RN) license in good standing.
  • Minimum of 2–5 years of nursing experience, preferably in home health care or community-based services.
  • Prior supervisory or leadership experience preferred.
  • Strong knowledge of home health regulations, care planning, and clinical documentation.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with electronic health records (EHR) and scheduling systems.

Work Schedule

  • Full-time position.
  • Required availability for on-call responsibilities, including evenings, weekends, and emergencies as needed.
  • Staff training responsibilities scheduled two days per week.
