Duties

  • Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patients’ health by taking vital signs, observing changes, and reporting findings accurately while adhering to HIPAA privacy standards
  • Assist with medication administration and manage care plans tailored to each individual's needs
  • Prepare nutritious meals and assist with meal planning in accordance with dietary requirements
  • Support patients with mobility using equipment such as Hoyer lifts or other assistive devices, ensuring safety during transfers and movement
  • Maintain a clean and safe environment by performing light housekeeping tasks like cleaning patient areas and laundry
  • Document patient progress, update care records, and communicate effectively with healthcare teams or family members

Skills

  • Proven experience in home health, assisted living, nursing homes, or group home settings is highly valued
  • Knowledge of patient observation techniques, dementia care (including Alzheimer's), memory care, and hospice support
  • Certification in CPR and First Aid; experience with HIPAA compliance is essential for maintaining confidentiality
  • Ability to work with individuals with developmental disabilities, autism spectrum disorder, or other special needs
  • Strong skills in assisting with ADLs, medication management, vital signs monitoring, and patient care documentation
  • Physical capability for heavy lifting and assisting with transfers using equipment like the Hoyer lift; stamina for long shifts required
  • Excellent communication skills for collaborating with clients’ families, healthcare providers, and team members
