Registered Nurse " Caregiver" evaluates clients referred to the registry and determines the type of services to be provided.

• Interviews the client and their family to determine appropriateness of services
• Obtains initial and ongoing referral documentation from physicians, insurers, and provides.
• Coordinates the development of the plan of care which will specifiy the frequency and duration of the service providers.
• Utilizes health record management software.

Qualifications

• Current State of Florida License or Multi-State License
• Proof of background screening including fingerprint check through the FBI & FDLE
• Professional Liability Insurance 
• State required continuing education units
• Social Security Card 
• Proof Identification
