Position Summary
The Clinical Case Worker will play a key role in coordinating care and services for clients enrolled in Targeted Case Management (TCM) and IOP programs. This individual will work closely with therapists, clinical supervisors, and community partners to ensure each client receives appropriate and timely support services. The ideal candidate is organized, client-centered, and familiar with community resources and behavioral health systems.
Key Responsibilities
Case Management & Client Support
- Provide strength-based, client-centered case management to children, adolescents, and families
- Develop and implement individualized service plans based on client needs and clinical recommendations
- Coordinate referrals for medical, psychiatric, housing, employment, and educational resources
- Maintain regular contact with clients to monitor progress and advocate for additional services
Collaboration & Communication
- Participate in interdisciplinary team meetings with therapists and IOP staff
- Support client transitions between levels of care, including IOP admissions and discharges
- Build and maintain partnerships with community agencies, schools, and support programs
Documentation & Compliance
- Maintain accurate, timely, and HIPAA-compliant case notes, assessments, and service documentation
- Ensure services align with Medicaid, managed care, and state documentation guidelines
- Assist with audits and quality assurance reviews as needed
Qualifications
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or related human services field
- 2+ years of experience in case management or community mental health setting
- Strong knowledge of TCM services, Medicaid, and local Dallas community resources
- Excellent organizational, communication, and time management skills
- Proficient in electronic health records and Microsoft Office Suite
- Bilingual (Spanish/English) preferred, but not required
- CPR, First Aid, and CPI certification preferred (or willingness to obtain)
