**Duties and Responsibilities:**

- Supervise individuals in their home environment.

- Assist individuals with goal-oriented life skills training.

- Foster meaningful relationships with individuals in care.

- Provide personal care assistance.

- Prepare and cook meals for individuals.

- Supervise and care for a member for a short period of time to relieve caregivers.

- Engage in community activities with individuals.

**Qualifications:**

- A minimum of three (3) months of experience in providing personal care and assistance to individuals.

- At least three (3) months of experience delivering habilitation services.

- Candidates must be 18 years of age or older.

- Ability to acquire an Arizona fingerprint clearance card is required.

- Successful completion of a criminal background check is mandatory.

- Provide contact information for three (3) professional references.

- Capability to lift and transfer individuals is necessary.

- Access to the internet and a telephone is required.

- Proficiency in general computer skills.

- Possession of a vehicle for transportation of individuals, including current registration and auto insurance.

*Please note: The sign-on bonus will be issued upon the successful completion of all training requirements, after 90 days of employment, and completion of 100 verified HCBS service hours.*
