We are seeking a Client Care Coordinator to join our growing team and play a key role in client intake, care coordination, and transportation scheduling.
Position Overview
The Client Care Coordinator serves as the primary point of contact for new and existing clients. This role is responsible for intake processing, onboarding clients into services, scheduling and tracking transportation, maintaining service documentation, and ensuring that all clients receive timely, supportive, and professional assistance.
This role requires exceptional communication, organization, and customer service skills, and must comply with Georgia DCH Private Home Care Provider rules (111-8-65) for documentation, client rights, and service coordination.
Key Responsibilities
- Conduct client intake, gather required documentation, and ensure accurate completion of all service forms.
- Coordinate client scheduling, transportation arrangements, and communication between caregivers and families.
- Maintain up-to-date client files, service logs, and transportation records in compliance with state regulations.
- Serve as the first point of contact for inquiries, service updates, and care-related concerns.
- Monitor client needs and preferences, communicating changes to management and caregiving staff.
- Ensure all client interactions are conducted with professionalism, confidentiality, and respect.
- Track daily attendance, service utilization, and transportation routes.
- Assist with care coordination tasks, including appointment reminders, travel logistics, and care follow-up.
- Support team members with administrative duties as needed.
- Uphold all agency policies related to client rights, safety, and quality of care.
Qualifications
- High school diploma required; associate’s or bachelor’s degree in human services, healthcare administration, social services, or related field preferred.
- Experience in home care, behavioral health, healthcare scheduling, case coordination, or administrative support strongly preferred.
- Excellent communication and customer-service skills.
- Strong organizational abilities with attention to detail.
- Ability to manage multiple tasks in a fast-paced environment.
- Basic computer skills (Microsoft Office, digital scheduling systems).
- Must pass background check, TB screening, and drug screening (as required by agency policy).
Preferred Skills
- Knowledge of respite care, home care services, or human services settings.
- Experience scheduling transportation or coordinating client logistics.
- Ability to handle sensitive and confidential information with discretion.
- Strong problem-solving skills and ability to remain calm under pressure.
