We are a growing Healthcare Management Services Organization (MSO) that partners with U.S.-based healthcare providers to streamline and manage their administrative operations. Our mission is to reduce the operational burden for our providers so they can focus on delivering high-quality patient care.

Duties and Responsibilities:

• Conduct telehealth visits (typically 99202/99203 level) for Medicaid patients seeking support with SDOH challenges
• Perform comprehensive clinical assessments to identify health needs and social barriers to care
• Provide medical clearance and referrals for CHW services to address access issues, home equipment needs, social supports, and care coordination
• Develop individualized care plans that address both clinical and social determinants of health
• Collaborate closely with Community Health Workers to support patient success
• Manage prescription refills and ongoing primary care needs as relationships develop
• Document visits thoroughly to meet state Medicaid program requirements
• Help patients navigate complex healthcare systems and connect to community resources

Minimum Qualifications

• Active Nurse Practitioner license with prescriptive authority
• Primary care experience in the state of CA, OK and HI (Family Practice, Internal Medicine, or similar)
• Experience working with Medicaid populations
• Strong understanding of social determinants of health and their impact on patient outcomes
• Comfortable with telehealth platforms and remote patient care
• Excellent communication skills and ability to build rapport virtually
• Knowledge of state Medicaid requirements for CHW referrals
