POSITION DESCRIPTION 

Position Title: Case Manager (Contractual) 

Supervisor: Program Director

Compensation: $50 - $100/Assessment & $25 - $40/hr for Case Management

Maryland License Requirement:  LPN / RN / NP / LBSW / LMSW / LCSW / LCSW-C


Position Function:  The Case Manager is a member of the multidisciplinary HIV Clinical Care Team and is responsible for managing a caseload, performing assessments/reassessments, submitting encounter notes and reports.  The Case Manager works directly with all ages of people living with HIV in accessing medical, psychological, and social services that are medically necessary and will positively impact the medical health outcomes, and their ability to remain in care and treatment.  This position includes both Diagnostic Evaluation Services and Ongoing Case Management throughout Maryland’s 23 counties and Baltimore City.  

 

Duties and Responsibilities: 

  • Facilitate access to intake, screening, and assessment for clients at the office, over the telephone, in a hospital, at the client’s home, or in other healthcare settings.
  • Understands the philosophy of person-centered planning and integrates this philosophy into creating the service planning process with the primary focus on treatment adherence and overall stability.
  • As applicable, complete assessments timely.
  • Ensures that Care Plans and other records are complete and accurate.
  • Co-develop comprehensive individualized Care Plans.
  • Coordinate services and activities required in implementing the Care Plan.
  • Liaise with other members of the client's treatment team.
  • Monitor and educate the client about medication side effects and lab results.
  • Address client’s financial and psychosocial barriers to receiving ongoing case management.
  • Complete encounter notes within 24 hours and ensures that all documentation contains descriptive, analytical, and evaluative content in the electronic health record system.
  • Maintain confidentiality, active Nurse/Social Worker state licensure, applicable background checks, and associated updates not limited to National Provider Identifier (NPI), Council for Affordable Quality Healthcare (CAQH), electronic Provider Revalidation and Enrollment Portal (ePrep), and credentialing for private/public health insurance. 
  • Participates in onboarding training and continual professional development.
  • Other duties as assigned.

 

Minimum Qualifications:

  • Associate’s degree in nursing or Master's Degree in Social Work preferred.
  • Active, unrestricted Maryland Nurse/Social Worker license.
  • Current CPR & First Aid Certification.
  • Valid NPI.
  • CAQH validation/re-validation.
  • Registered in ePrep.
  • Current commercial liability insurance.
  • Valid and unrestricted driver’s license.
  • Access to private transportation.


Experience: 

  • At least one (1) year providing Case Management.
  • At least one (1) year in providing sexual health education and health issues impacting PLWHA.

 

Knowledge and Skills: 

  • Ability to engage with all staff and clients with respect.  Should excel at working in a fast-paced environment.  
  • Working knowledge of the nursing process and critical thinking abilities are

required.

  • Knowledge of electronic health records and tracking systems is required.
  • Must be able to type at least 35 words per minute.
  • Fluency in English. Bilingual speakers are welcome.
  • Excellent judgment in clinical settings.
  • Excellent communication skills with staff and providers.
  • Knowledge of principles of nurse case management.
  • Ability to use electronic health record systems.
  • Maintains confidentiality of patients at all times.
  • Proficiency with Microsoft Outlook, Word, Excel, and PowerPoint.
  • Strong organizational skills with the ability to prioritize projects, and meet deadlines.
  • Ability to work independently and as part of a team.
  • Good judgment, problem-solving, and decision-making skills.

 

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:

  • Use standard office equipment and databases to input and retrieve information.
  • Communicate effectively in person or via telephone.
  • Perform complex mental functions; collect, interpret, and/or analyze complex data and information.
  • Coordinate multiple tasks simultaneously.
  • Must be able to sit or stand continuously for a minimum of one hour and move to and from the work area multiple times during the workday.
  • Lift up to 20 pounds.
