Job Overview
The Home Health Physical Therapist is responsible for providing skilled physical therapy services to patients in their place of residence. This role focuses on restoring function, improving mobility, relieving pain, and preventing disability through individualized, goal-oriented physical therapy interventions. The Physical Therapist collaborates with physicians, patients, families, and interdisciplinary team members to ensure high-quality, patient-centered care and optimal clinical outcomes.
Responsibilities / Essential Functions
Clinical Care & Assessment
- Receive and review intake referral information and prepare all physical therapy documentation and tools required for patient visits
- Perform comprehensive physical therapy evaluations and reassessments, documenting findings accurately and within required timeframes
- Conduct home safety and environmental assessments and implement interventions to reduce fall risk and improve safety
- Assist the physician in evaluating functional status through diagnostic and prognostic muscle, nerve, joint, and functional testing
Plan of Care & Treatment
- Develop, implement, and update individualized Physical Therapy Plans of Care (POC) based on patient needs and physician orders
- Submit Plans of Care and recommendations to the physician for review, approval, and ongoing coordination
- Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence
- Evaluate Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) and incorporate findings into treatment planning
- Determine medical necessity for continued therapy and for referral to other disciplines as appropriate
Communication & Coordination
- Maintain effective communication with patients and families regarding treatment goals, progress, and outcomes
- Communicate with physicians verbally and/or in writing regarding patient status, treatment recommendations, and therapy effectiveness
- Collaborate with interdisciplinary team members to coordinate care and ensure continuity of services, including participation in case conferences as scheduled or triggered by clinical events
- Provide education and instruction to patients and caregivers on the safe use of prosthetic, orthotic, and assistive devices (e.g., canes, walkers, wheelchairs, mechanical lifts)
Supervision & Compliance
- Develop and implement the Plan of Care when PT is the only skilled discipline ordered
- Complete and submit all required OASIS assessments and documentation within 48 hours of the OASIS visit
- Participate in Clinical Record Reviews (CRR) in collaboration with leadership
Minimum Qualifications
- Current, active California Physical Therapy license in good standing
- Minimum of two (2) years of experience as a Physical Therapist, in a home health setting
- Successful completion of a criminal background check
- Current CPR/BLS certification
- Valid California driver's license, reliable personal vehicle, and current automobile insurance
- Proficiency with electronic medical record (EMR) systems and mobile documentation tools
