Job Overview


HealingHaven Home Healthcare is a home health agency committed to delivering compassionate, high-quality care. We are seeking a dedicated Home Health Occupational Therapist to join our growing clinical team and help patients maximize independence, safety, and functional outcomes.


Responsibilities / Essential Functions

Clinical Care & Assessment

  • Perform comprehensive occupational therapy evaluations and reassessments, documenting findings accurately and within required timeframes
  • Obtain and document pertinent medical, functional, and psychosocial history from patients, families, and/or caregivers
  • Conduct home safety and environmental assessments and implement interventions to reduce fall risk and improve safety
  • Evaluate patient performance in ADLs and iADLs and incorporate findings into treatment planning
  • Complete OASIS assessments accurately and submit within 48 hours of the OASIS visit


Plan of Care & Treatment

  • Develop, implement, and revise individualized Occupational Therapy Plans of Care based on patient needs and physician orders
  • Submit Plans of Care and recommendations to the physician for review, approval, and ongoing coordination
  • Provide skilled therapeutic interventions to improve functional independence, safety, strength, and cognitive performance
  • Establish measurable OT goals based on patient function and rehabilitation potential


Communication & Coordination

  • Maintain effective communication with patients and families regarding treatment goals, progress, and outcomes
  • Communicate with physicians verbally and/or in writing regarding patient status, treatment recommendations, and therapy effectiveness
  • Collaborate with interdisciplinary team members to coordinate care and ensure continuity of services, including participation in case conferences as scheduled or triggered by clinical events


Supervision & Compliance

  • Develop and implement the Plan of Care when OT is the only skilled discipline ordered
  • Supervise HHAs in accordance with federal and state regulations when OT is the primary discipline
  • Participate in Clinical Record Reviews (CRR) in collaboration with leadership as required


Quality Improvement & Professional Development

  • Participate in Performance Improvement (PI/QAPI) activities, committees, and initiatives as assigned
  • Attend and actively participate in staff meetings, in-services, case conferences, and required trainings
  • Maintain current professional licensure and pursue continuing education to enhance clinical competence and patient outcomes


Skills & Attributes

  • Strong clinical skills in ADL/iADL training, functional mobility, safety interventions, and adaptive equipment
  • Knowledge of home health regulations and standards (CMS Conditions of Participation, CDPH, ACHC)
  • Excellent communication and interpersonal skills


Minimum Qualifications

  • Current, active California Occupational Therapist license in good standing
  • Minimum of two (2) years of experience as an Occupational Therapist, in a home health setting
  • Successful completion of a criminal background check
  • Current CPR/BLS certification
  • Valid California driver's license, reliable personal vehicle, and current automobile insurance
  • Proficiency with electronic medical record (EMR) systems and mobile documentation tools

Salary

$60 - $150 per hour
