Job Description:

Position Overview The Licensed Vocational Nurse (LVN) provides skilled nursing services under the supervision of a Registered Nurse (RN) in accordance with the patient's plan of care. This role involves direct patient care, patient/family education, and close collaboration with the interdisciplinary team.

Key Responsibilities

• Provide skilled nursing visits in patient homes per plan of care
• Perform focused assessments and report changes in condition to supervising RN
• Execute treatments and procedures using proper clinical techniques
• Educate patients and caregivers on disease management, medications, and self-care
• Reinforce care plans and collaborate with RN case managers on patient progress
• Maintain accurate, timely, and compliant documentation
• Participate in case conferences and interdisciplinary communication
• Identify safety risks in the home and report concerns appropriately
• Support continuity of care and patient engagement

Qualifications

• Active California LVN license
• Minimum 2 years LVN experience in Home Health
• Current CPR certification
• Understand OASIS and EMR Charting in home health industry
• Ability to work independently in a field-based environment
• Reliable transportation and valid driver's license
