JOB DESCRIPTION
The behavioral health care manager is a crucial member of the collaborative care team, working closely with the patient’s medical provider and psychiatric consultant, as well as the larger primary care team. They are responsible for supporting and coordinating the mental and physical health care of patients on their assigned caseload, in collaboration with the medical provider and other mental health professionals as needed.
DUTIES AND RESPONSIBILITIES
1. Assist in the mental and physical health care of patients on the assigned caseload, coordinating care with the medical provider and other mental health professionals when necessary.
2. Conduct screenings and assessments for common mental health and substance abuse disorders, and facilitate patient engagement and follow-up care.
3. Educate patients on common mental health and substance abuse disorders, as well as available treatment options.
4. Monitor treatment response and track patients for changes in symptoms or treatment complications, either in person or over the phone.
5. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, side effects, and effectiveness.
6. Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, and motivational interviewing.
7. Arrange referrals for evidence-based psychosocial treatments as needed, both within the clinic and through outside providers.
8. Participate in regular caseload consultations with the psychiatric consultant, discussing treatment recommendations with the medical provider.
9. Track patient follow-up and outcomes using a registry system, documenting encounters and utilizing the system to re-engage patients as needed.
10. Document patient progress and treatment recommendations in the EHR and other required systems for sharing with medical providers and other treating professionals.
11. Collaborate with the medical provider and psychiatric consultant to make treatment plan changes for patients who are not improving as expected.
12. Coordinate referrals for additional services outside of the organization, such as social services, vocational rehabilitation, and specialty mental health care.
13. Develop relapse prevention self-management plans for patients nearing discharge from the caseload.
14. Cross-train for in-office procedures, including rooming patients, collecting lab specimens, blood draws, and vital signs.
TRAINING & LICENSURE REQUIREMENTS
▪ Registered Nurse (BSN recommended)
PREFERRED EXPERIENCE
· Experience with children and adolescents
· At least one year of experience in an integrated health model.
