About the Role:

Are you a dedicated and detail-oriented Nurse looking for flexibility and meaningful work in the community? Join our Adult Foster Care team as a Pay Per Visit Clinician and play a critical role in delivering personalized care to elderly and disabled members in their homes.

This independent contractor role offers flexible scheduling, competitive per-visit pay, and the opportunity to collaborate with a multidisciplinary team (MDT) to support member health and well-being.

What You’ll Do:

·      ✅ Monthly Visits

- Conduct scheduled monthly member visits

- Submit complete visit notes within 24 hours

- Ensure accurate ADL documentation and member signatures in care logs.


·      ✅ Semi-Annual Health Status Reports

- Complete detailed reports every 6 months

- Coordinate with Primary Care Providers (PCPs)

- Ensure compliance and required signatures


·      ✅ Plan of Care Management

- Develop and update individualized Plans of Care

- Adjust based on member conditions and progress


·      ✅ Annual MDS Review

- Review Minimum Data Set (MDS) annually

- Update Plan of Care; submit for approval


·      ✅ Monthly MDT Meetings

- Participate in case review meetings

- Present summaries and update Plans of Care

What We’re Looking For:

- Licensed Nurse in Massachusetts

- Home care or community-based nursing experience preferred

- Strong documentation and time management skills

- Reliable transportation for in-home visits

- Familiarity with MyUnity or similar EMR systems is a plus
