The Member Specialist plays a critical role in supporting members and caregivers through the onboarding process and ongoing engagement in homecare programs. The Member Specialist plays a critical role in supporting members and caregivers through the onboarding process and ongoing engagement in homecare programs. The ideal candidate is empathetic, organized, and proactive — able to build trust with members while coordinating services and ensuring compliance with regulatory and agency standards.
· Develop and communicate clearly onboarding processes for prospective members.
· Ensure timely and complete submission of applications and supporting documentation.
· Conduct home visits as needed to support member intake and care planning.
· Track and report completion of annual member requirements (Care Plans, Semi-Annuals, Physicals).
· Document, monitor and execute on member discharge requests and conduct required follow up.
· Facilitate communication between members, caregivers, and interdisciplinary teams.
· Develop and maintain up to date directories of community resources, by territory, available to support members and their caregivers.
· Educate members and staff about relevant community resources and services.
· Lead or coordinate caregiver education and training programs in collaboration with the Caregiver Specialist.
· Respond to member crises and emergency needs with urgency and compassion.
· Conduct six to ten weekly quality control calls to a selected number of members.
· Conduct two to three visits per month to member’s home to assess their satisfaction with GIA services.
· Accurately document services and interactions in MyUnity within required timelines.
· Support interdepartmental coordination through weekly huddles and collaborative problem-solving.
· Evaluate service delivery and recommend process improvements.
· Conduct annual member satisfaction surveys, compile results, and develop and submit processes, service and policies that reflect survey feedback.
$22 - $24 per hour
