The Care Manager plays a vital role in coordinating the psychosocial and logistical care of members receiving Adult Foster Care services. This includes assessing home settings, developing care plans, supporting caregivers, and ensuring regulatory compliance. The role requires compassion, attention to detail, and collaborative problem-solving with the care team and member families.
· Conduct initial and ongoing psychosocial evaluations of members for AFC eligibility.
· Participate in developing interim and final care plans with the MDT and family members.
· Ensure implementation and monitoring of AFC plans of care.
· Conduct on-site visits at qualified member settings per AFC level requirements.
· Complete progress notes for each on-site visit or significant change in condition.
· Conduct annual review of the safety and suitability of each care setting.
· Select, train, evaluate, and supervise AFC caregivers.
· Review caregiver logs monthly and provide support as needed.
· Provide education and training to ensure compliance and quality of care.
· Coordinate services with healthcare or community providers (e.g., VNA, DDS, MRC).
· Refer members to external services when needed.
· Participate in discharge and transition planning.
· Respond to urgent or emergency needs promptly.
· Collaborate with MDT and caregivers to develop emergency backup care plans.
