The Case Coordinator serves as a point of contact for participants, families, and/or clients performing related duties toward the objectives and goals.

As a Case Manager, you'll build meaningful relationships with children, families, schools, providers, and community partners while helping connect people to the services and resources they need.


This is more than paperwork and referrals. You'll help families understand their options, coordinate support, identify barriers, and advocate for the people we serve.


What You'll Do

Every family has different needs. You'll work collaboratively to understand those needs, coordinate services, and help create a path forward.

  • Provide ongoing case management and service coordination
  • Build positive, trusting relationships with individuals and families
  • Gather information from families, schools, medical providers, and community agencies
  • Help families understand available programs, resources, and services
  • Screen referrals and connect families to appropriate services
  • Coordinate communication between families, providers, agencies, and other professionals
  • Identify gaps or barriers in services and help develop solutions
  • Advocate for the needs of individuals and families
  • Provide education, support, and technical assistance to families, providers, and community agencies
  • Develop and maintain strong relationships with community partners
  • Attend team, case, agency, and other required meetings
  • Maintain accurate, confidential case records and documentation
  • Prepare reports and other documentation related to case management and service delivery
  • Maintain documentation required for court proceedings when applicable
  • Complete required documentation accurately and within three business days

Qualifications

  • Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
  • Social Work certification or credential is desirable
  • Two or more years of experience in health, human services, case management, or a related field preferred
  • Experience working with children, individuals with disabilities, and/or families is a plus
  • Valid driver's license required
  • Ability to travel between locations and appointments as needed
  • Strong communication and interpersonal skills
  • Excellent organization and time-management skills
  • Ability to manage multiple cases, priorities, deadlines, and documentation requirements
  • Strong attention to detail and written communication skills
  • Ability to build professional relationships with families and community partners
  • Ability to work independently while also functioning effectively as part of a team
