Organization: Free Baltimore

Location: Baltimore, MD (On-site/Community-based)

Target Population: Youth and Adults

Schedule: Monday–Friday (Full-Time)

Position Summary

Free Baltimore is seeking a dedicated and resilient Case Manager to support community members navigating the complexities of the justice system. This role is central to our mission of reducing recidivism and fostering personal independence. You will manage a high-volume schedule that includes one-on-one advocacy for adults and facilitating group sessions for both youth and adults weekly. The ideal candidate possesses a deep understanding of trauma-informed care and the systemic barriers faced by individuals in the Baltimore region.


Key Responsibilities

Individual Case Management (Adults)

  • Conduct weekly one-on-one sessions for an adult caseload, focusing on personal, social, and vocational stability.
  • Perform comprehensive Rehabilitation & Needs Assessments to identify barriers related to mental health, substance use, housing, and reentry.
  • Develop and monitor Individualized Service Plans (ISPs) that promote self-sufficiency and emotional regulation.
  • Provide crisis intervention and de-escalation as needed.

Group Facilitation (Youth & Adults)

  • Lead group sessions weekly utilizing evidence-based curricula (e.g., Cognitive Behavioral Interventions, or Life Skills.
  • Facilitate youth-centered groups focused on decision-making, conflict resolution, and educational engagement.
  • Manage adult group dynamics to address workforce readiness, restorative justice principles, and community reintegration.

Advocacy & Coordination

  • Maintain meticulous case files and progress notes in compliance with state and organizational standards.
  • Collaborate with the legal system: Work alongside probation officers, attorneys, and community partners to ensure a holistic support network.
  • Navigate Baltimore’s social service landscape to connect clients with housing, healthcare, and specialized vocational training.

Qualifications

  • Education: Bachelor’s Degree in Social Work (BSW), Psychology, Counseling, Criminal Justice, or a related field required. Bachelor's or Master’s Degree (MSW/MA) preferred.
  • Experience: * Minimum of 2 years working within the criminal justice system or with high-risk populations.
  • Proven experience in group facilitation and managing group dynamics.
  • Deep familiarity with the socio-economic landscape and community resources of Baltimore City.
  • Skills: * Proficiency in Trauma-Informed Care and Motivational Interviewing.
  • Bilingual (Spanish/English) is a significant plus.
  • Strong "street-smarts" and the ability to maintain professional boundaries in high-stress environments.
  • Excellent verbal/written communication and proficiency in EHR/Case Management software.

Salary & Benefits

  • Salary Range: $60,000 – $63,000 per year (Commensurate with experience).
