We are looking for compassionate, dependable, and purpose-driven individuals to join our team as Registered Nurse Care Navigator. The RN Care Navigator provides high-touch nursing assessment, education, planning, care coordination, and follow-through for private-pay clients. The Navigator learns the client's needs and goals, helps organize healthcare information and next steps, communicates with authorized family members and providers, and responds to reported changes. This role supports non-medical home care and does not replace a physician, emergency services, or provider-ordered skilled home health, therapy, or other medical treatment
Responsibilities
- Complete RN assessments and scheduled navigation visits; identify client goals, risks, barriers, support needs, and appropriate next steps within the RN's lawful scope and agency service agreement.
- Review available discharge instructions, medication lists, warning signs, follow-up appointments, referrals, tests, equipment, home-safety concerns, mobility concerns, and personal-care needs.
- Prepare clients for healthcare appointments; attend appointments when requested, authorized, scheduled, and included in the service agreement; document instructions and help organize follow-through.
- With valid authorization, communicate relevant observations and client-reported information to physicians, specialists, pharmacies, home health, therapy, hospice, and other providers; route clinical questions and medication discrepancies to the appropriate treating clinician or pharmacist.
- Track provider-directed medication changes, tests, referrals, instructions, and unresolved next steps without prescribing, independently changing medications, or administering medications under this non-medical service.
- Follow up after reported changes in condition, emergency-department visits, hospitalizations, or facility discharges; determine whether an additional RN navigation assessment is appropriate and escalate urgent concerns according to agency policy.
- Provide understandable education to clients and authorized caregivers regarding diagnoses, instructions, warning signs, safety, care options, and when to contact a provider or 911.
- Keep the client and authorized family informed through timely, objective communication while honoring privacy, consent, client choice, and professional boundaries.
- Support the personal-care team by helping develop client-specific caregiver instructions and by receiving caregiver reports of decline or concern; caregivers observe and report, while the RN evaluates and coordinates the response.
- Document assessments, contacts, education, coordination, reported changes, actions, and follow-up accurately and promptly in the agency record.
- Participate in quality improvement, incident review, emergency preparedness, staff education, competency activities, and other duties consistent with licensure and agency policy.
What We Are Looking For
- Current, active, unencumbered Oklahoma Registered Nurse license.
- At least two years of relevant nursing experience preferred; geriatric, hospice, home care, care-management, discharge-planning, or chronic-illness experience strongly preferred.
- Current CPR certification and other health, screening, background, and employment requirements established by law and agency policy.
- Knowledge of nursing scope, confidentiality, client rights, abuse/neglect reporting, change-in-condition response, documentation, and community resources.
- Reliable transportation, valid driver's license, required automobile insurance, working phone, and ability to travel within the service area.
Preferred Experience
Experience is valuable, but character matters more.
Preferred backgrounds include:
- Previoius care navigation/coordination experience
- Senior living, assisted living, hospice, hospital, or nursing facility experience
- Experience caring for a loved one at home
- Dementia or Alzheimer’s care experience
