Description
- Conduct initial assessments and intake interviews with new residents to determine their needs and develop individualized care plans
- Monitor and evaluate residents' progress, adjusting care plans as necessary
- Provide ongoing support and counseling to residents to address their physical, emotional, and social needs
- Coordinate with healthcare providers, social workers, and other professionals to ensure comprehensive care
- Maintain accurate and up-to-date case files and documentation
- Assist residents in accessing community resources such as education, employment, and recreational programs
- Develop and facilitate life skills training sessions for residents
- Advocate for residents' rights and needs within the community and service systems
- Ensure compliance with all relevant regulations and organizational policies
- Participate in team meetings, case conferences, and training sessions
Residential Case Manager Qualifications & Skills
- Bachelor's degree in social work, psychology, or a related field
- Previous experience in residential case management or a similar role
- Excellent interpersonal and communication skills
- Ability to work effectively in a team environment
- Knowledge of community resources and services
- Proficient in case management software and Microsoft Office applications
- Bachelor's degree in social work, psychology, human services, or a related field
- Minimum of 2 years of experience in case management or a related field
- Strong organizational and time management skills
- Ability to maintain confidentiality and professional boundaries
- Valid driver's license and reliable transportation
- CPR and First Aid certification
- Ability to work flexible hours, including evenings and weekends, as needed
- Strong problem-solving skills and the ability to make sound decisions under pressure
- Empathy, patience, and a genuine desire to help others
Salary
$20 - $20 per hour
