Clinical Case Managers play an integral role in the delivery of clinical services and facilitating client’s access to critical supports and resources by helping to reduce barriers to treatment and keeping program participants engaged in addiction treatment and recovery support services.
Qualifications:
- Must have a minimum of five (5) years experience in recovery coaching and/or recovery coordination with individuals in recovery from substance use disorders, specifically with youth or adults.
- Ability to effectively communicate with executive leadership, colleagues, and other stakeholders.
- Must meet all staff requirements, including background screening and drug testing.
- Demonstrate professionalism, confidentiality, good judgment, and cultural competence in providing services.
- Proficient in verbal, written, and interpersonal communication skills.
- Familiarity with Office 365, CIMOR, COMBAT Connections, and other required data systems.
- Knowledge of community resources and programs to support program participants.
Duties and Responsibilities:
- Assess clients' treatment needs, develop and monitor treatment plans, and evaluate progress.
- Conduct intake interviews and assessments for new clients.
- Determine treatment goals and coordinate services to address client needs.
- Monitor and advocate for clients, providing personal support and intervening in crises.
- Maintain accurate client records and communicate progress to the therapeutic team and family.
- Provide educational resources to staff and evaluate the application of learning to treatment results.
- Implement changes to improve treatment outcomes and meet budget projections.
- Prepare reports on treatment data and trends, compiling statistics and completing grant reports.
- Stay updated on job knowledge by participating in educational opportunities and professional organizations.
- Collaborate in service-delivery/team staffing meetings to ensure effective communication on treatment goals and progress.
Other duties as assigned by supervisor.
$18 - $23 per hour
