The Housing Case Manager is responsible for providing comprehensive case management services to homeless asylum seekers temporarily housed in a hotel. This role is crucial in ensuring that clients receive the necessary support and resources to transition into stable housing.

The Housing Case Manager is responsible for coordinating services and resources that contribute directly to the well-being and stability of clients.
Client Assessment: Conduct thorough assessments of clients' needs and develop individualized service plans.
Resource Coordination: Connect clients with appropriate community resources, including housing, healthcare, employment services, and other needed services.
Advocacy: Advocate on behalf of clients to ensure access to necessary services and support.
Documentation: Maintain accurate and up-to-date records of client interactions and progress.
Client Engagement: Maintain regular contact with clients to monitor progress and adjust service plans as needed.
Outcome Achievement: Successfully transition clients from temporary housing to stable, long-term housing solutions.
Education: Bachelor's degree in Social Work, Human Services, or related field preferred
Experience: Previous experience in case management or social services, particularly with homeless populations
Certifications: None required, but relevant certifications are a plus
This role is based in a hotel setting with standard working hours, and in the office. Occasional travel to community resource locations may be required.
$25 - $26 per hour
