Position Overview
The RN Nurse is responsible for coordinating and conducting client intake assessments, developing individualized care plans, and ensuring the delivery of high-quality nursing services to clients in their homes. This role requires both administrative and clinical expertise, with a strong focus on intake coordination, regulatory compliance, and providing hands-on care as needed.
Key Responsibilities
Intake & Care Coordination
- Conduct initial client intake assessments, including health history, medication review, and evaluation of care needs.
- Develop and implement individualized care plans in collaboration with the client, family, and care team.
- Serve as the primary nursing contact during the admission process, ensuring smooth onboarding for clients and families.
- Coordinate with physicians, social workers, and other healthcare providers to ensure continuity of care.
- Document all intake findings, care plans, and updates accurately in the agency’s EMR system (Alora Plus).
Clinical Duties
- Performing skilled nursing services
- Provide education to clients, families, and caregivers on disease management, safety, and health maintenance.
- Conduct supervisory visits to ensure caregivers follow care plans and provide quality services.
- Respond to client health concerns, incidents, and emergencies, following agency protocols.
- Maintain compliance with state and federal regulations
Quality Assurance & Compliance
- Complete documentation in a timely, accurate, and compliant manner.
- Participate in audits, case reviews, and quality improvement initiatives.
- Report incidents, medication errors, and client condition changes per agency policy.
