Position Summery

The Nurse Coordinator plays a vital role in ensuring day-to-day clinical operations run smoothly. This position supports the Nurse Manager in assessments, care documentation, client file maintenance, and the delegation/training of direct care staff.

This is an excellent opportunity for a nurse who enjoys balancing field work with administrative coordination and is passionate about working in home and community-based care settings.

Key Responsibilities

  • Assist with completing initial and ongoing RN assessments for 245D clients
  • Prepare and maintain client documentation, including CSSP Addendums and health plans
  • Coordinate medication setup, reviews, and delegation of nursing tasks
  • Maintain client files and ensure timely updates for audits and compliance
  • Schedule and assist in training DSPs on delegated tasks, infection control, and emergency procedures
  • Communicate with case managers, families, and pharmacies regarding client care
  • Monitor incident reports and follow up as needed
  • Help track expiration dates for client and staff certifications, medications, or service plans
  • Provide on-call support (optional or rotating if applicable)

Qualifications

  • Registered Nurse (RN) in Minnesota
  • Minimum 1 year of experience in home care, group homes, or community-based services
  • Understanding of HCBS 245D, Basic Home Care, or disability support services
  • Strong organization, documentation, and follow-through skills
  • Comfortable using computers and/or EHR systems
  • Reliable transportation and valid driver’s license

Salary
