Assist in providing supported services based on an individual support plans and desired activities in promoting community life.
Assist with planning, organizing and participating in activities provided to individuals in the home and community.
Provide transportation within the community utilizing personal vehicle.
Meet the needs, desires and activities of the individual we support.
Ability to establish effective working relationships with co-workers, supervisors, families, and others who are essential for the well-being of the individual's we support.
Ability to respond to emergency situations appropriately.
The work environment for this position varies and is a community-based locations providing support to the individuals and their family in their home and in the community.
Knowledge of health measures such as basic first aid, CPR, fire safety and personal safety measures.
Minimum Requirements
Valid driver's license with daily access to a privately maintained and insured vehicle
At least 18 years of age
Effective communication skills both verbal and written.
Successfully completion of required training as mandated by regulation.
Successfully completed preemployment process including motor vehicle record and criminal background check.
