Key Responsibilities

• Provide direct patient care in accordance with physician orders and RN supervision.
• Monitor vital signs and patient condition, reporting changes promptly.
• Administer medications and treatments safely, per scope of practice.
• Assist with wound care, trach/ventilator management, G-tube feeding, and other specialized procedures (as trained and permitted by licensure).
• Educate families and caregivers on daily care routines, safety measures, and equipment use.
• Document care accurately and in compliance with agency and regulatory standards.
• Collaborate with RNs, CNAs, PCAs, and other care team members to deliver coordinated care.
• Maintain infection control standards and ensure a safe environment in the home.
• Participate in ongoing training, skills validation, and professional development.
