• Provide skilled care including bathing, dressing, grooming, toileting, feeding, and mobility support.
• Monitor client condition and report any changes—physically, emotionally, or behaviorally—to RN.
• Support safe medication routines as directed by RN or PCA.
• Assist in implementing clients’ individualized service plans.
• Maintain accurate and timely documentation of care provided.
• Uphold infection control standards and ensure a clean, safe environment.
• Participate in orientation and in-service training.
• Collaborate with the RN, PCAs, families, and other care team members for coordinated, compassionate care.
