Our Interdisciplinary Team functions as a family unit to assist each patient and their loved ones during this journey of life. Each member of our team can be reached anytime seven days a week. Hospice Teams are Comprised of Registered Nurses and Licensed Vocational Nurses Certified Home Health Aides Licensed Social Workers Certified Spiritual Care Coordinators Certified Bereavement Counselors Volunteers Hospice Medical Director The Hospice benefit is 100% covered by Medicare A, Medicaid, and some private insurances. The benefit includes the provision of medications related to the terminal diagnosis, nursing care, counseling services, medical equipment, and medical supplies, dietary consultation along with therapy consultation for comfort to include Physical, Speech, or Occupational. Patients and families have the right to request Hospice services and select their hospice provider. Consultations can be arranged directly by the family. Job Summary:

We are seeking a compassionate and highly skilled On-Call Registered Nurse (RN) to join our hospice/home health team. This role provides after-hours and weekend coverage, ensuring that our patients receive quality care and timely response to urgent needs. The RN will be responsible for triaging calls, managing weekend admissions, responding to patient deaths, and coordinating with full-time case managers for continuity of care.


Key Responsibilities:

  • Triage and Respond to Calls: Provide after-hours support for patient and family needs, assessing and prioritizing calls for appropriate action.
  • Weekend Admissions: Conduct hospice admissions as needed during on-call shifts, ensuring timely and compassionate patient onboarding.
  • Pronouncement of Deaths: Respond to and document patient deaths with professionalism and empathy, following hospice protocols.
  • Coordination with Case Managers: Communicate patient updates and reports to full-time case managers to ensure seamless transitions of care.
  • Symptom Management: Provide guidance and nursing interventions for symptom control, pain management, and crisis situations.
  • Family and Caregiver Support: Offer education and emotional support to families and caregivers during critical situations.
  • Documentation & Compliance: Maintain accurate and timely electronic health records (EHR) following agency and regulatory guidelines.


Qualifications & Requirements:

  • Active RN license in the state of Texas.
  • Minimum 1-2 years of hospice or home health experience preferred.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Ability to work independently and respond effectively in urgent situations.
  • Excellent communication and interpersonal skills.
  • Proficiency in using electronic medical records (EMR) systems.
  • Valid driver’s license and reliable transportation.
