Position Summary:

We are seeking an experienced and organized Licensed Vocational Nurse (LVN) to join our team as an Office Intake and Scheduling Specialist. This role is vital for ensuring seamless intake and coordination of care for our patients. The ideal candidate will have strong communication skills, the ability to work collaboratively, and a keen attention to detail to support timely and efficient patient care.


Key Responsibilities:

  • Conduct patient intake and assessment processes, ensuring all necessary documentation is complete and accurate.
  • Coordinate and schedule patient visits, ensuring appropriate staff availability and efficient routing for field staff.
  • Verify insurance eligibility and benefits, coordinating with billing and authorization teams as needed.
  • Serve as a liaison between patients, families, healthcare providers, and internal staff to ensure clear communication and timely response.
  • Document patient information accurately in the electronic medical record system.
  • Assist in identifying patient needs and communicating these to clinical and administrative teams.
  • Maintain confidentiality and adhere to all HIPAA regulations and guidelines.


Qualifications:

  • Valid LVN license in [State].
  • At least 1-2 years of experience in home health, hospice, or a healthcare office setting.
  • Knowledge of medical terminology, home health, and hospice processes is highly preferred.
  • Strong organizational skills with a keen attention to detail.
  • Proficiency in electronic medical records (EMR) and scheduling software. (Kinnser and Axxess)
  • Exceptional interpersonal skills to interact with patients, families, and healthcare professionals.
  • Proficiency in Outlook, Office 365, Excel, Teams, Word, Adobe and Powerpoint.
  • Ability to multitask in a fast-paced environment and prioritize workload.
