Per visit home health RN position in the DFW
As a PRN- Home Health Registered Nurse, you will have the opportunity to make a positive impact on the lives of patients in the comfort of their own homes.
**Job Overview:**
As a per visit Home Health Registered Nurse, you will be responsible for providing high-quality and personalized care to patients in need of home health services. You will work closely with patients, their families, and a multidisciplinary team to develop and implement individualized care plans that promote wellness and independence. This is a part-time position offering flexible hours and competitive compensation ranging from $35 to $40 per hour (onboarding). Once onboarding is completed you will be paid at a per visit rate for the following:
Nursing (Medicare Rates)
RN OASIS SOC = $130
RN OASIS REC = $100
RN OASIS ROC = $85
RN OASIS DC = $80
RN Visit = $60
Nursing (Insurance Rates)
RN OASIS SOC = $120
RN OASIS REC = $90
RN OASIS ROC = $80
RN OASIS DC = $75
RN Visit = $55
**Responsibilities:**
- Conduct comprehensive assessments of patients' health status and develop personalized care plans in collaboration with the patient, their families, and the healthcare team.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patients' progress, document observations, and communicate any changes in patients' condition to the healthcare team.
- Educate patients and their families on self-care techniques, medication administration, and disease management.
- Collaborate with other healthcare professionals to coordinate and ensure continuity of care.
- Provide emotional support and compassionate care to patients and their families.
- Maintain accurate and up-to-date medical records and documentation.

- Valid Registered Nurse (RN) license in the state of [location].

We are looking for a Registered Nurse to join our Home Health team. The ideal candidate will have experience in acute care, outpatient, long term care, rehabilitation center, and/or assisted living facility settings. The successful candidate will be responsible for providing quality care to patients in their homes, assessing patient needs and developing care plans, administering medications and treatments, and providing education to patients and their families.
