The Mental Health Provider is responsible for delivering community-based, trauma-informed therapeutic services to individuals experiencing homelessness under the Continuum of Care (CoC) program. This role provides integrated mental health support in coordination with housing services to promote long-term housing stability and overall wellness. The provider will maintain an active caseload and work collaboratively with a multidisciplinary team to support clients with complex needs.

Key Responsibilities


Direct Clinical Services:

  • Maintain an active caseload of approximately 20 clients.
  • Provide community-based, trauma-informed individual therapy to clients experiencing homelessness.
  • Utilize evidence-based practices such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and harm-reduction approaches.

Integrated Care Coordination:

  • Work closely with housing staff to integrate mental health services into housing stability plans.
  • Participate in regular case conferences with Housing Case Managers, Peer Support Specialists, and Addiction Specialists to coordinate care.

Client Assessment & Treatment Planning:

  • Conduct comprehensive behavioral health assessments to identify client needs and barriers to housing stability.
  • Develop and implement individualized treatment plans with measurable goals.
  • Monitor client progress and update treatment plans as needed.

Community-Based Engagement:

  • Deliver services in community settings, including client homes, shelters, and other approved locations.
  • Support clients in building skills necessary for long-term housing stability, particularly within transitional housing programs.

Documentation & Compliance:

  • Maintain accurate, timely, and compliant clinical documentation in the agency’s case management system.
  • Ensure all services align with CoC grant requirements and agency policies.

Linkage to Services:

  • Connect clients to additional supports, including psychiatric care, substance use treatment, and community resources.

Program Support:

  • Support program operations and administrative needs as required.
  • Perform other duties as assigned to support the housing and behavioral health programs.


Qualifications

  • Education: Master’s degree in Social Work, Counseling, or a related behavioral health field.
  • Licensure: Current or provisional licensure in North Carolina (e.g., LCSW-A, LCSW, LCMHC-A, LCMHC, LCAS, or equivalent).
  • Experience: Minimum of 2 years of experience providing mental health services, preferably with individuals experiencing homelessness, trauma, or co-occurring disorders.
  • Other Requirements: Valid driver’s license and reliable transportation for community-based work.

Salary

$75,000 - $80,000 per year
