Key Responsibilities
Direct Clinical Services:
- Maintain an active caseload of approximately 20 clients.
- Provide community-based, trauma-informed individual therapy to clients experiencing homelessness.
- Utilize evidence-based practices such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and harm-reduction approaches.
Integrated Care Coordination:
- Work closely with housing staff to integrate mental health services into housing stability plans.
- Participate in regular case conferences with Housing Case Managers, Peer Support Specialists, and Addiction Specialists to coordinate care.
Client Assessment & Treatment Planning:
- Conduct comprehensive behavioral health assessments to identify client needs and barriers to housing stability.
- Develop and implement individualized treatment plans with measurable goals.
- Monitor client progress and update treatment plans as needed.
Community-Based Engagement:
- Deliver services in community settings, including client homes, shelters, and other approved locations.
- Support clients in building skills necessary for long-term housing stability, particularly within transitional housing programs.
Documentation & Compliance:
- Maintain accurate, timely, and compliant clinical documentation in the agency’s case management system.
- Ensure all services align with CoC grant requirements and agency policies.
Linkage to Services:
- Connect clients to additional supports, including psychiatric care, substance use treatment, and community resources.
Program Support:
- Support program operations and administrative needs as required.
- Perform other duties as assigned to support the housing and behavioral health programs.
Qualifications
- Education: Master’s degree in Social Work, Counseling, or a related behavioral health field.
- Licensure: Current or provisional licensure in North Carolina (e.g., LCSW-A, LCSW, LCMHC-A, LCMHC, LCAS, or equivalent).
- Experience: Minimum of 2 years of experience providing mental health services, preferably with individuals experiencing homelessness, trauma, or co-occurring disorders.
- Other Requirements: Valid driver’s license and reliable transportation for community-based work.
