To provide overall medical direction for Care Chexx LLC, specifically in a recuperative care facility serving homeless adult males and females; to deliver direct medical services to clients; to provide clinical supervision, consultation, and collaboration with physicians, physician assistants, nurse practitioners, nursing staff and social workers; to consult and collaborate with organizational staff, managers, and other healthcare personnel; to act as a liaison with community health and medical organizations; and to perform related duties as assigned.


Examples of Work Performed

As the Medical Director at Care Chexx LLC, you will play a pivotal role in promoting a diverse, culturally competent, and respectful workplace. Your responsibilities will include organizing, planning, delegating, and directing the medical activities of our recuperative care facility. This includes overseeing medical respite services, patient discharge planning, and infectious disease control.

 

In addition, you will provide direct client care and consultation within the facility, develop and approve standing orders and protocols tailored to the needs of homeless adult males and females, and provide medical direction and consultation for emergency preparedness specific to the facility.

 

You will also be responsible for establishing medical priorities and designing innovative service programs to address the complex needs of the homeless population, providing clinical supervision and consultation to nursing staff, and facilitating training opportunities for medical residents and healthcare personnel.

 

Furthermore, you will serve as a professional liaison between Care Chexx LLC and local hospitals, health systems, partners, and community clinics, provide leadership and guidance to community-based health providers, state agencies, and partners, and collaborate with local, state, and federal health agencies to support medical and operational objectives.

 

You will contribute to the development of Care Chexx LLC's annual budget and participate as a key member of the organization's leadership team, submitting reports and recommendations to the Chief Administrative Officer or designee, and coordinating with laboratory services to ensure quality standards and compliance.


Minimum Qualifications

Education:

  • Master’s Degree or higher in Public Health.
  • Completion of post-graduate medical training in preventive medicine, internal medicine/pediatrics, or family practice with successful board certification.
  • Must be in good standing with medical board and successfully pass criminal and DHS background.


Experience:

  • Two (2) years of advanced professional experience in internal medicine, preventive medicine, or family practice.


Certification/Licensure/Requirements:

  • Valid medical license in the State of Minnesota and maintenance of continuing medical education requirements.
  • Must be registered with Minnesota Health Care Programs (MHCP) as a licensed independent provider or can apply for enrollment.


Supervisory/Managerial Responsibility:

  • Provide clinical supervision to medical personnel as required by law.
  • Serve as a consulting physician to Advanced Practice Nurses and oversee credentialing for medical staff.


Work Environment:

  • Work is conducted primarily in a recuperative care facility for homeless adults, with occasional home/shelter visits.
  • Virtual and in-person consultations with medical partners, hospitals, community organizations and state and federal agencies.
  • Direct contact with individuals experiencing homelessness, including those with infectious or other complex health conditions. Care Chexx personnel must complete a baseline individual TB risk assessment, symptom screening, and TB testing (e.g., TB skin test or TB blood test) upon hire/pre-placement.
  •  Care Chexx workforce members are recommended to be fully vaccinated and boosted against COVID-19.
  • Care Chexx workforce members are recommended to be fully vaccinated against Hepatitis B upon hire with (2 dose series of Heplisav-B with doses separated by at least 30 days). All persons refusing vaccination must sign disclosure of refusal.

  • Regularly engage with clients, staff, external medical providers, and community organizations.
  • Collaborate with public health entities and other agencies to ensure quality and efficiency in service delivery.

Salary

$60.18 - $74.83 per hour
