The Brighter Futures case manager will play a crucial role in supporting older youth aging out of foster care. This position involves providing comprehensive case management services, including assessment, planning, coordination, and advocacy, to ensure that program participants have the resources and support they need to transition successfully to independent living. The Case Manager will work closely with youth, caregivers, community partners, and other stakeholders to develop individualized service plans and connect participants with essential resources.

Description

Qualifications

  • Minimum of Bachelor's degree in Social Work, Psychology, Counseling, or a related field required. Candidates should have a history of demonstrating strong skills in the position's key responsibilities. 
  • Minimum of 2 years of experience in child welfare.
  • Experience with older youth in foster care or transitional living programs is highly desirable. 
  • Excellent written and verbal communication skills.
  • Proficiency in Google Suite, Microsoft Office, and data management software.

Responsibilities

Case Management

  • Conduct thorough assessments of participants' needs, strengths, and goals.
  • Develop and implement individualized service plans in collaboration with participants.
  • Provide ongoing support and monitoring to ensure participants' progress toward their goals.
  • Maintain accurate and up-to-date case records and documentation.

Resource Coordination

  • Connect participants with community resources, including housing, employment, education, healthcare, and life skills training.
  • Facilitate access to financial assistance programs and other support services.
  • Advocate for participants' needs and rights within various systems and agencies.

Relationship Building

  • Build trusting and supportive relationships with program participants.
  • Engage with caregivers, family members, and other support networks to enhance participants' stability and well-being.
  • Collaborate with CASA volunteers, DFCS case managers, attorneys, and service providers to create a robust network of support for participants.

Program Development

  • Participate in the development and implementation of program policies, procedures, and best practices.
  • Contribute to program evaluation and continuous improvement efforts.
  • Assist in the development of outreach and recruitment strategies to engage eligible youth.

  • Occasionally, it includes reaching above shoulder height or below the waist or lifting to 25 pounds for office equipment, supplies, and materials.


Salary

$44,000 - $49,000 per year
