Position Summary
The Intake & Case Manager will support BMHV’s clinical department and community programming by serving as a first point of contact for participants seeking services, completing intake and assessment processes, coordinating referrals, and providing short-term case management support. This role will also help pilot BMHV’s technology systems for intake, referral tracking, case coordination, and data capture so the organization can strengthen service delivery and better understand community needs.
The position bridges clinical and community-based healing work. The person in this role will help participants move across BMHV’s internal healing pathways, including clinical services and other healing departments, based on participant goals and interest.
Core Responsibilities
Intake and participant onboarding
- Serve as a welcoming first point of contact for individuals, families, and community members seeking BMHV services.
- Conduct intake interviews by phone, virtually, and in person as needed.
- Complete intake forms, releases, consent documentation, screenings, and service eligibility steps in alignment with organizational policies.
- Explain BMHV service options clearly and help participants identify the supports that best fit their needs.
Case management and care coordination
- Provide short-term case management and care coordination for participants connected to clinical and community programs.
- Develop and track individualized support plans tied to participant-identified goals.
- Coordinate warm handoffs to therapists, support groups, healing circles, youth programs, and other internal BMHV offerings.
- Connect participants to external community resources when needs fall outside BMHV’s direct services.
- Maintain follow-up contact to reduce drop-off and support continuity of care.
Internal referral pipeline support
- Help build and maintain an internal referral pipeline across BMHV healing departments.
- Work with program and clinical staff to ensure participants can move across services smoothly and with informed consent.
- Flag barriers, delays, and service gaps that affect participant access or engagement.
Technology and data systems pilot
- Use BMHV’s technology platform(s) to document intakes, referrals, service connections, follow-up activity, and outcomes.
- Pilot workflows for intake and case management technology and provide feedback on usability, equity, and operational fit.
- Support accurate, timely, and complete data entry across assigned cases and workflows.
- Assist with data quality review and basic reporting needs to help BMHV strengthen program learning and impact storytelling.
Collaboration and administration
- Participate in team meetings, supervision, case review, and workflow improvement discussions.
- Collaborate with clinical, youth, and community programming staff to coordinate participant care and improve systems.
- Maintain confidential records and uphold ethical, professional, and culturally responsive practice at all times.
- Support occasional evening or weekend community activities when needed.
Qualifications
Required
- Bachelor’s degree in social work, psychology, counseling, public health, human services, or a related field, or equivalent relevant professional experience.
- At least 1–2 years of experience in intake, case management, care coordination, community health, mental health, youth development, or related direct-service work.
- Strong organizational skills and ability to manage documentation, follow-up, and participant communication consistently.
- Comfort using digital tools such as Google Workspace and web-based case management, CRM, or referral systems.
- Demonstrated commitment to Black mental health, healing-centered engagement, equity, and culturally responsive practice.
Preferred
- 3 or more years of relevant experience in nonprofit, community mental health, or youth-serving environments.
- Experience supporting clinical intake or behavioral health referral processes.
- Familiarity with HIPAA-conscious documentation, consent practices, and confidentiality standards.
- Experience piloting, improving, or troubleshooting program systems and workflows.
- Knowledge of Nashville-area health, social service, and community resources.

Schedule and work environment
This is a full-time, exempt position based in Nashville, Tennessee. The role may include a mix of in-person, community-based, and administrative work depending on program and participant needs.
