Responsibilities

• Perform in-home assessments and ongoing health monitoring
• Deliver skilled nursing procedures: wound care, IV therapy, catheter care, injections, G/J tube care, tracheostomy and ventilator management
• Provide life-sustaining interventions for high-risk patients (e.g., ventilator-dependent)
• Document care using Therap (EHR system) and follow established care plans
• Collaborate with families, case managers, and providers for coordinated care
• Offer patient/family education on self-care, medication management, and recovery

Requirements

• Active RN license in the state of North Dakota
• CPR certification (required)
• Recent TB test or screening
• Prior home care or private duty nursing experience (preferred)
• Reliable transportation and willingness to travel to client homes
• Strong communication and documentation skills
