Description
Responsibilities
• Conduct initial patient evaluations and develop individualized plans of care.
• Provide and oversee skilled nursing care, ensuring best practices are followed.
• Regularly assess, revise, and update care plans as patient needs evolve.
• Document clinical notes, progress summaries, and patient records accurately.
• Educate and counsel patients and families on health needs and care plans.
• Supervise and mentor nursing staff, providing training and in-service education.
• Ensure compliance with HIPAA, OSHA, and state/federal regulations.
• Collaborate with physicians, therapists, and caregivers to ensure continuity of care.
• Promote the agency’s mission and represent Berachah in the healthcare community.
Requirements
• Active RN license in the state of Texas (unencumbered).
• Minimum 2–5 years of clinical nursing experience; at least 1 year in home health preferred.
• Proven ability to supervise, train, and mentor nursing staff.
• Strong patient assessment, care planning, and case management skills.
• Excellent communication, leadership, and organizational abilities.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Must pass a background check.
Preferred Qualifications
• Bachelor’s Degree in Nursing (BSN) from an accredited program.
• Experience in home health licensure compliance and regulatory standards.
• Knowledge of OSHA, HIPAA, and Medicare/Medicaid requirements.
• Skilled in case management and policy development.
