Position Summary:
Battle Creek Family Care, is seeking a dedicated and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to provide comprehensive, high-quality healthcare to our ambulatory population. This position involves direct patient care, care coordination, preventative health education, and active collaboration with an interdisciplinary team.
The ideal candidate will be a strong patient advocate, an effective communicator, and passionate about delivering patient-centered care in a community-focused setting.
Key Responsibilities:
- Provide direct primary care services to patients in an outpatient setting
- Conduct comprehensive health evaluations and develop individualized care plans
- Assess patients’ psychosocial, emotional, physiological, and environmental needs
- Collaborate with physicians, nurses, and support staff to ensure integrated care
- Participate in case management, referrals, and interdisciplinary care conferences
- Deliver health education and counseling to patients and families
- Promote disease prevention and wellness through patient engagement and education
- Participate in quality improvement initiatives, including protocol development, chart audits, and data analysis
- Stay current with best practices through continuing education and participation in professional organizations
- Utilize ancillary services and prescription management with a focus on cost-effectiveness
- Engage in research and apply evidence-based practices to patient care
Minimum Qualifications:
For Nurse Practitioner (NP):
- Master of Science in Nursing (MSN)
- Graduation from an accredited Nurse Practitioner program in a relevant specialty
- Current licensure as an NP in the state of Michigan
- National certification in the respective specialty (preferred)
For Physician Assistant (PA):
- Graduate of an accredited Physician Assistant program
- Certified or eligible for certification by the National Commission on Certification of Physician Assistants (NCCPA)
- Licensed or eligible for licensure in the state of Michigan
Skills & Competencies:
- Strong clinical assessment and diagnostic capabilities
- Excellent interpersonal, verbal, and written communication skills
- Proven ability to work effectively in a team-based, multidisciplinary environment
- Commitment to providing culturally competent and inclusive care
- Proficient in using electronic health records (EHR)
- Understanding of healthcare quality improvement and patient safety best practices
