What You’ll Do
As a Licensed Discharge Planner you will play a key role in creating an environment where patients and families feel supported, safe, and inspired to grow. You will:
- Work directly with the Director of Client and Community Experience on tasks that support the expansion of BasePoint’s community partnerships through appropriate discharge planning.
- Collaborate with clinic staff regarding recommendation for discharge planning
- Assist families with connecting to resources, and ensure follow up appointments are made prior to discharge.
- Complete family consultation to assist in parent knowledge of step-down care and provide appropriate information
- Act as a point of contact for the Outreach Team regarding any Patient or account needs
- Clearly communicate the status of current projects to the Director of Client and Community Experience
- Support in development of Alumni programming, and provide clinical services within program
- Facilitate follow up care for all patients, at a 30-, 60- and 90-day rate with appropriate tracking of follow up care and beyond day 90 for alumni involved patients
- Provide quality customer service to families and agencies

What We’re Looking For:
- Applicants must hold a master’s degree or equivalent
- Licensed Clinical Social Worker, Licensed Master’s Social Worker, Licensed Professional Counselor, Licensed Professional Counselor - Associate.
- (Licensure can be pending)
- MSW (Agreement to be Licensed within 6 months of start date)
- 1-2 years’ experience in mental/behavioral health
