The Care Coordinator for Pathways CCSO Services supports youth and families through trauma-informed, individualized care coordination. This role involves building relationships, leading care team meetings, coordinating services, and ensuring each family's voice drives the care plan.

Description

Essential Job Duties and Responsibilities

  • Provide client- and family-driven care in alignment with the Pathways Care Model, maintaining appropriate caseloads.
  • Coordinate and facilitate services, including Child and Family Team (CFT) meetings, to support safety, crisis prevention, and individualized needs.
  • Engage families and supports through in-home visits, phone, email, and other methods of contact.
  • Collaborate with Managed Care Organizations, providers, and the mobile crisis team to ensure continuity and access to care.
  • Complete accurate and timely documentation in compliance with organizational, state, and federal requirements.

 

Qualifications:

  • Requires current standing as a Mental Health Professional credential in the State of Illinois
  • Bachelor’s degree in a behavioral/social science field from an accredited college or university preferred
  • Prior experience providing direct services in case management or care coordination services in accordance with State of Illinois Medicaid criteria preferred. 
  • Familiarity and willingness to learn and work with computer-based systems
  • Excellent written and verbal communication skills
  • Bi-lingual Spanish/English preferred


Compensation and Benefits:

  • Pay Range: $21.29-$31.93 per hour

  • Day shift with occasional evenings
