Description

Essential Responsibilities

·  Conduct initial and periodic nursing assessments, as assigned, to identify client needs, risks, functional status, and changes in condition.

·  Develop, review, and update individualized service or care plans in collaboration with the client, family, CEO, and other authorized members of the care team.

·  Conduct supervisory and quality-assurance visits as assigned; evaluate whether non-medical services are delivered safely, consistently, and in accordance with the approved care plan.

·  Observe client status and promptly communicate significant changes, safety concerns, or potential medical needs to the CEO and the appropriate authorized provider or emergency resource.

·  Provide education, competency validation, and in-service training to Care Partners on topics within the nurse's scope, including infection prevention, fall prevention, recognizing changes in condition, emergency procedures, and client-specific support needs.

·  Review client records, incident reports, and service documentation for accuracy, completeness, timeliness, and compliance; recommend corrective action when needed.

·  Support medication-safety education and review medication information for care planning purposes; Care Partners may provide reminders only and may not administer medications unless legally authorized.

·  Participate in authorized care conferences and coordinate with families, providers, hospitals, home health, hospice, and community partners; support infection-control, emergency-preparedness, quality-improvement, and safety activities.

·  Maintain client confidentiality and comply with HIPAA, company policies, professional standards, and applicable federal, state, and local requirements.

·  Maintain accurate, timely nursing documentation and submit all required reports according to company procedures.

·  Remain available on an as-needed basis for assigned assessments, visits, consultations, training, urgent clinical guidance, meetings, and professional development.

·  Perform other duties within the RN scope of practice as assigned to support client safety and the operational needs of Amee's Legacy Elevated Care.

 

Scope of Role

·  Amee's Legacy Elevated Care is a non-medical home care provider. This position primarily provides clinical oversight, assessment, care-planning support, education, consultation, and quality assurance.

·  The PRN Registered Nurse will not independently provide skilled nursing treatment, medication administration, wound care, injections, or other billable clinical services unless such services are separately authorized by the company, permitted by law, included in an approved service arrangement, and supported by required orders and documentation.

 

Qualifications

·  Current, active, and unrestricted Registered Nurse license in the State of Alabama.

·  Associate or bachelor's degree in nursing from an accredited nursing program; Bachelor of Science in Nursing preferred.

·  Minimum of two years of nursing experience; home care, home health, geriatrics, hospice, rehabilitation, or community-based care experience preferred.

·  Current CPR/BLS certification.

·  Knowledge of nursing assessment, care planning, infection prevention, client safety, documentation, and professional standards.

·  Ability to pass a criminal background check, drug screening, and any other required health or employment screening.

·  Valid driver's license, reliable transportation, current automobile insurance, and ability to travel to assigned client homes and service locations.

·  Excellent communication, teaching, organization, critical-thinking, and customer-service skills.

·  Ability to work independently, exercise sound professional judgment, respond appropriately to urgent concerns, and use electronic documentation and basic office technology.

 

Physical and Work Requirements

·  Ability to stand, walk, bend, reach, and sit for extended periods while completing assessments, training, and documentation.

·  Ability to lift or assist with up to 25 pounds and to demonstrate safe client-support techniques, with reasonable accommodation when applicable.

·  Ability to work in private residences where pets, stairs, or other household conditions may be present; PRN scheduling may include weekdays, evenings, weekends, or urgent assignments.
