POSITION SUMMARY:

The Case Manager is responsible for providing comprehensive case management services to individuals and families in need of support. This includes conducting assessments, developing individualized care plans, coordinating services, advocating for client needs, and monitoring progress to promote client stability, well-being, and self-sufficiency.

 

KEY RESPONSIBILITIES:

  • Conduct bio-psychosocial assessments and develop individualized service or treatment plans.
  • Coordinate referrals and link clients to community resources including housing, mental health, medical, educational, and employment services.
  • Monitor client progress through regular contact and adjust service plans as needed.
  • Advocate on behalf of clients with service providers, landlords, schools, and government agencies.
  • Maintain accurate, timely, and confidential case records in accordance with program standards and regulatory requirements.
  • Participate in multidisciplinary team meetings and case consultations.
  • Assist with crisis intervention and safety planning as necessary.
  • Educate clients and families about available services, rights, and responsibilities.

 

REQUIRED QUALIFICATIONS:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
  • Minimum of 1–3 years of relevant experience in case management or social services
  • Knowledge of local community resources and service systems
  • Knowledge of family systems theory, trauma-informed care, and evidence-based treatment modalities.
  • Proven ability to work effectively with children and adolescents, as well as diverse populations and trauma-impacted clients.
  • Strong communication, documentation, and interpersonal skills


PREFERRED QUALIFICATIONS:

  • Master’s degree in social work, Counseling, or a related field
  • Experience working with populations served
  • Familiarity with relevant regulations and documentation standards
  • Bilingual or multilingual fluency.

 

WORK ENVIRONMENT:

  • Work is primarily conducted onsite at the agency or in the community
  • May require occasional evening or weekend hours to meet client needs.
  • May involve moderate travel within service area.

LICENSURE SUPERVISION OPPORTUNITIES:

We offer supervision for eligible candidates working toward licensure under the guidance of a licensed clinical social worker (LCSW), licensed professional counselor (LPC), depending on licensure track.

Salary

$29 - $42 per hour
