Job Details:

• Number of Positions: 1
• Location: Columbia, South Carolina
• Duration: 11/20/23 - 2/25/23 with possible extension/conversion
• Pay: $16/hr

Duties:

• 50% Provides health coaching or intervention telephonically or in person for assigned health programs. Utilizes all data systems containing patient information and documents information appropriately.
• 30% Assists with the identification of areas for which there is a need for health management programs. Monitors quality indicators and develops programs to positively impact areas where improvement is warranted. Develops patient/provider educational materials.
• 10% Assists with implementation of health management activities/programs for membership. Facilitates the program to participants and focuses on medium-risk patients.
• 10% Assists with the development of strategies to increase the effectiveness of and participation in health management programs.

Skills and Requirements:

• Ability to work independently and prioritize effectively
• Good judgment skills
• Strong customer service and organizational skills
• Proficiency in Microsoft Word, Excel, PowerPoint
• Ability to handle confidential or sensitive information with discretion
• 3 years of clinical experience
• Associate Degree in Medical Assisting OR completion of a post-secondary Medical Assistant Program
• Certified Medical Assistant through AAMA, AMA, or NRCMA

Languages:

• English (Speak, Read, Write)

Schedule Notes:

• Partially Onsite (Mainly Remote, but will need to be onsite the first week and occasional travel)
• Work hours: 8 am to 5 pm Monday to Friday
• Hours Per Week: 40.00
• Hours Per Day: 8.00
