We are seeking a dedicated and detail-oriented Service Coordinator to join our team. The Service Coordinator will act as a liaison between participants, caregivers, and service providers, ensuring that individuals receive the services and supports they need to thrive. This role involves developing and monitoring service plans, coordinating resources, and advocating for participants’ needs.

Description

  • Act as the participant's advocate, provide unbiased assistance to the participant with the selection of providers, and where appropriate, assist the participant in locating a place to live in the community
  • Assist clients/ legal guardians with applications and paperwork for and management of benefits, financial, housing, or other related areas as needed.
  • Provide each participant a Waiver Contact List, as well as provide a review of this Contact List to assure it is correct, up-to-date and redistributed on an as-needed basis
  • Provide assistance to waiver participants in terms of implementing the plan, as well as reviewing its effectiveness
  • Coordination and scheduling of waiver and non-waiver services
  • Maintain consistent and open communication with the RRDS and waiver participants
  • Assure timely implementation of the approved service plan
  • Formally review and submit a revised service plan as directed by applicable Waiver.
  • Maintain client records for a minimum of 7 years after termination of Waiver services from Amazing Home Care.
  • Ensure the assessment of need for a nursing facility level of care is completed on an annual basis.
  • Conduct face-to-face meetings with the waiver participant at least monthly to monitor health and welfare needs as he/she adjusts to the waiver program
  • Schedule team meetings 6 weeks prior to the end of the most recently approved Service Plan period and/or on an as needed basis and provide written summaries of the Team Meetings to all waiver providers
  • Maintain case records consistent with all contacts and necessary documentation
  • Maintain a tracking system that monitors all areas of compliance
  • Practices proper safety procedures and take precautions at all times in order to ensure their safety, as well as the safety of their coworkers
  • Reports all safety hazards or concerns to their supervisor in a timely fashion
  • Comply with all company policies and procedures

Qualifications:


• Experience in case management, social work, or a related field.

• Strong communication, organizational, and interpersonal skills.

• Familiarity with community resources and support programs.

• Commitment to supporting individuals with diverse needs.


Associates Degree with 5 years minimum healthcare experience

Bachelor's degree with at least a year of experience in healthcare

Salary

$72,000 - $90,000 per year
