Abundance of Love Home Healthcare, LLC is seeking a compassionate and qualified Medical Social Worker to provide PRN medical social work services to patients and families receiving care in their homes. The Medical Social Worker will work as part of an interdisciplinary care team to assess and address social, emotional, environmental and socioeconomic factors that may affect a patient's health, ability to cope with illness or disability, and ability to remain safely and successfully at home. This position is well suited for a social worker who enjoys working independently, building meaningful relationships with patients and families, and connecting individuals with appropriate healthcare, social-service and community resources.

Description

Key Responsibilities

Patient Assessment & Medical Social Work Services

  • Conduct initial and ongoing psychosocial assessments based on patient needs and authorized services.
  • Assess social, emotional, environmental and socioeconomic factors that may affect the patient's health, daily functioning or ability to cope with illness or disability.
  • Identify barriers that may interfere with the patient's ability to obtain maximum benefit from medical care.
  • Provide supportive and rehabilitative social work services within the individual's scope of practice and the patient's plan of treatment.
  • Help patients and families understand, accept and follow appropriate medical recommendations.
  • Assist patients and families with personal, social and environmental difficulties that may affect health, safety or well-being.
  • Help patients and families identify and access appropriate family, healthcare and community resources.
  • Provide information, referrals and assistance with available social-service programs and community resources.
  • Support patients and families in adapting to illness, disability and changes in their living circumstances.


Care Coordination & Interdisciplinary Collaboration

  • Participate in the development and implementation of the interdisciplinary plan of care as it relates to medical social work.
  • Collaborate with the Agency Supervisor, nurses, physicians and other members of the healthcare team.
  • Assist members of the healthcare team in understanding social, emotional and environmental factors affecting the patient's health and care.
  • Observe, document and report significant changes in the patient's social or emotional status.
  • Communicate concerns that may affect the patient's health, safety, ability to remain at home or response to treatment.
  • Consult with the attending physician and other healthcare professionals regarding changes that may require modification of the plan of treatment.
  • Coordinate social work visits with other agency services to support effective patient care.
  • Participate in admission and discharge planning as appropriate.
  • Evaluate the patient's and family's response to social work interventions and modify interventions as appropriate.


Documentation & Professional Practice

  • Prepare timely, accurate and complete clinical and progress notes for the patient's medical record.
  • Document assessments, interventions, referrals, patient/family responses and follow-up activities.
  • Maintain accurate clinical documentation in accordance with agency policies and requirements.
  • Maintain patient confidentiality and professional boundaries.
  • Comply with agency policies, procedures and applicable professional standards.
  • Participate in agency meetings, staff development activities and in-service education as appropriate.
  • Maintain professional knowledge and skills through continuing education, workshops, conferences, professional organizations and independent study.


Qualifications

Required

  • Bachelor's or master's degree in Social Work from an accredited program.
  • Current, unrestricted Illinois Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license in good standing, as applicable to the services provided.
  • Ability to provide medical social work services in accordance with the patient's plan of treatment and applicable scope of practice.
  • Strong interpersonal, communication, assessment and documentation skills.
  • Ability to work independently in patients' homes.
  • Ability to communicate effectively, both verbally and in writing.
  • Reliable transportation and ability to travel to patient homes within the agency's service area.
  • Ability to access and work safely within patients' homes, including homes that may not be routinely wheelchair accessible.
  • Current CPR certification. Online-only CPR certification is not accepted.
  • Successful completion of required background screening and employment checks.
  • Ability to meet agency health-screening and credentialing requirements.

Preferred

  • Previous medical social work experience.
  • Home nursing, home health, hospice or other community-based healthcare experience.
  • Experience working with medically complex patients and their families.
  • Experience conducting psychosocial assessments and participating in interdisciplinary care planning.
  • Familiarity with Illinois community resources and social-service programs.
  • Experience working with patients and families coping with chronic illness, disability or significant changes in functional status.
  • Experience working as part of an interdisciplinary healthcare team.
  • LCSW credential.


Work Environment & Physical Requirements

This is a community-based position requiring travel to patients' homes.

The position requires the ability to:

  • Walk, sit, stand and stoop as necessary while providing services in the home.
  • Navigate patient homes and environments that may have limited accessibility.
  • Communicate effectively with patients, families and healthcare professionals.
  • Perform the essential functions of the position safely and effectively.
  • Tolerate the physical demands associated with working in a variety of home environments.
