Our clients are generally very treatment-resistant, and our clinical focus is harm reduction, so it's critical that our staff be able to connect with our clients and speak their language.
Responsibilities:
- Oversees coordination of client needs and works in collaboration with client and primary therapist in treatment/discharge planning and goals, identifying additional needed support services including educational, vocational, legal, recreational, mental health, and social services
- Completes and maintains all documentation of services delivered
- Ensures cohesion of services documented within client files, and assists with compliance with DHCS and Joint Commission standards
- Keeps a schedule of consistent case management sessions with clients
- Coordinates service provision with probation officers, attorneys, etc., as needed
- Complies with local, state, and federal confidentiality laws and regulations
- Facilitates group counseling sessions
- Collaborates with members of the treatment team to ensure the effective delivery of a full spectrum of clinical and social services
- Attends and participates in weekly treatment team meetings
- Participates in staff meetings when appropriate
Requirements:

MUST be located in the state of North Carolina
- Must be able to accurately document all services provided to clients
- Ability to develop excellent rapport with clients
- Must be able to communicate effectively, both orally and in writing
- Must have the ability to coordinate a continuum of treatment/services for clients
- Must have a high school diploma or equivalent
- Must be certified with RAD-T, SUDCC, CAADE, CATC, CCAPP, CADC or CADTP OR willing to get certification within 14 days of hire
