In-Home Assessment Nurse – RN
360 Case Management is seeking a dependable, compassionate, and detail-oriented Registered Nurse (RN) to complete in-home nursing assessments for individuals receiving services through Georgia Medicaid Waiver Programs and for private-pay care management clients throughout the Central Savannah River Area (CSRA).
This position is ideal for an experienced nurse who enjoys working independently in the community, completing comprehensive assessments, identifying client needs, and collaborating with case management staff to help individuals remain safely in their homes.
Service Area
This position serves clients throughout the CSRA, including Augusta and surrounding Georgia communities.
Travel to participants’ homes within the assigned service area is required.
Compensation
$150 per completed assessment
Payment is based on the successful completion and submission of the required assessment and supporting documentation.
Position Responsibilities
The In-Home Assessment Nurse will:
- Conduct scheduled nursing assessments in participants’ homes throughout the CSRA.
- Complete comprehensive assessments of the participant’s current health, functional abilities, safety, and care needs.
- Review medical diagnoses, medications, treatments, recent hospitalizations, and relevant medical history.
- Assess activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Evaluate mobility, transfers, fall risk, cognition, nutrition, skin integrity, continence, and other health-related needs.
- Identify potential health and safety concerns within the home environment.
- Assess the participant’s current informal and formal support system.
- Identify changes in condition or circumstances that may require additional services or follow-up.
- Obtain and document vital signs and other appropriate nursing observations when required.
- Complete required assessment forms and supporting documentation accurately and within established deadlines.
- Communicate significant findings or concerns promptly to the assigned Case Manager or Program Manager.
- Collaborate with case management staff regarding recommendations and identified participant needs.
- Complete initial assessments, reassessments, and other nursing evaluations as assigned.
- Follow applicable Georgia Medicaid Waiver requirements, agency policies, HIPAA requirements, and professional nursing standards.
- Maintain professional and respectful communication with participants, families, authorized representatives, caregivers, and agency staff.
Private-Pay Assessments
The nurse may also conduct in-home assessments for private-pay clients to assist with:
- Determining current care and supervision needs
- Identifying risks and safety concerns
- Evaluating the client’s ability to remain safely at home
- Assessing caregiver and family support needs
- Developing recommendations for care management services
- Identifying the potential need for additional medical, personal care, community, or supportive services
Minimum Qualifications
- Current and unrestricted Registered Nurse (RN) license
- Eligible to practice nursing in the State of Georgia
- Current CPR/BLS certification, if required for assigned services
- Valid driver’s license
- Reliable transportation and ability to travel throughout the CSRA
- Ability to work independently in community settings
- Strong assessment, documentation, communication, and organizational skills
- Ability to complete electronic documentation
- Ability to maintain participant confidentiality and comply with HIPAA requirements
Preferred Qualifications
- Previous home health, case management, Medicaid waiver, long-term care, geriatric, disability, or community nursing experience
- Experience completing comprehensive in-home nursing assessments
- Familiarity with Georgia Medicaid Waiver Programs, including EDWP, SOURCE, CCSP, or similar community-based programs
- Experience working with older adults and individuals with physical disabilities or chronic medical conditions
Schedule
This is a flexible, assessment-based position serving the CSRA. Assessments are scheduled according to participant needs, geographic location, and nurse availability.
Nurses must be available to travel to assigned participants’ residences and submit completed documentation within established agency timelines.
Compensation
$150 for each completed and accepted in-home assessment.
Additional assignments may become available based on program growth, geographic coverage, and assessment volume.
