Description

A licensed nurse will visit the client's residence to conduct a thorough assessment for the Elderly and Disabled Waiver Program. This evaluation is crucial for determining the level of services required, as informed by the MDSHC (Maximum Daily Service Hours Criteria) assessment. The healthcare professional must hold an active license as either a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Georgia. Additionally, this role necessitates travel across several counties, specifically Jefferson, Wilkes, Washington, and Lincoln counties.
