NextCare Providers LLC is an Arizona-based home health agency dedicated to providing compassionate, high-quality, patient-centered care to individuals and families in the comfort of their homes, including individuals receiving services through Arizona’s Division of Developmental Disabilities (DDD). Our mission is to provide individualized care that promotes independence, dignity, safety, and an improved quality of life for the people we serve. Our care model is centered around one-on-one care and strong collaboration between our field clinicians and internal teams. While our nurses often work independently in the field, we believe they should never feel like they are working alone. Our established support structure includes our Director of Nursing and clinical leadership for clinical guidance and escalation, Scheduling for patient assignments and coverage, Quality Assurance for documentation and compliance support, and Human Resources and Administration for onboarding, credentials, training, and employee needs. We also provide ongoing nursing in-service education, patient-specific training and shadowing when appropriate, and electronic systems that help keep our field and internal teams connected.
